Care to elaborate on what the noise is?

there's a lot things rotating in there, If this was my car and had to work it out myself and I could not isolate it I'd try a process of elimination. Maybe remove the main drive belt and see if it goes away completely or a significant change.
You've got bearings in the alternator, A/C unit, 2 tensioner pulley bearings, pwr steering pump, then under the timing cover more pulleys, belt tensioner and a water pump.
hard to tell with all the other noises but could be a very dry worn out bearing. Could be just the main belt tensioner pulley.
If it's within the timing area I'd give it some priority. A 1994 galant that I salvaged and brought back to life had it's engine terminated due to poor maintenance practices. Every bearing was shot, one had literaly exploded and separated taking out the timing belt, 19 valves and a badly impacted piston. Even the water pump was bound up.
I know for my magna, all seals, bearings and tensioner were done with the belt , water pump will get done next time if I still have it.
I have had rumbly vibration noises due to under tensioned drive belt and also have had a separated harmonic balancer.

Thanks so much for the great reply TW2005, I bought the car off a guy at work and it was very poorly maintained, he is someone that waits till something fails before he would fix it, he hates spending money. Anyway back to the car, it only makes that noise when the motor is under load and idling, in neutral it doesn't do it. its done 275,000 k's

Definitely transmission related by the sounds of it

Could be the baffle in the "Y" exhaust pipe just behind engine, it only rattles when cold, and the transmission is selected to drive or reverse causing the engine to vibrate enough to shake the broken baffle. there is a thread earlier about noisy exhaust with photos. Search: "noisy exhaust baffle" , forum page 2, line 10, wackywheelz 24-10-2012 1:37p.m. Good description with photos.
